Report tip or complaint

If you are dissatisfied, we appreciate you discussing this with us. If you are dissatisfied with any of our practice staff, please report it to us. If we are aware of your dissatisfaction, we can try to resolve it for you. Your complaint will be investigated and possibly discussed with you so that we can learn from it.

If you find it difficult to report it over the phone, it is also possible to do so in writing. You can use the complaint form, which you can collect from the reception desk or here download. Once we receive the completed form, we will start working on your complaint. We will contact you as soon as possible to see how we can deal with your complaint. There may be a meeting with the staff member involved or a consultation with a third party. We then determine what we can do for you and implement the required actions. We assume that this will enable us to settle the complaint to your satisfaction. In the unlikely event that this is not the case, you can take your complaint to an independent complaints officer. We also submit complicated complaints to this committee.

As part of the WKKGZ Act, as a practice we have joined DOKH's complaints desk. You will then be put in touch with an independent and impartial DOKH complaints officer. You will have an interview with the complaints officer, who will support you throughout the process. You will receive advice and you can come here with all your questions. Once the complaint has been clearly described, you will work together with the complaints officer to find a solution. This is usually followed by a meeting between you, the staff member and the complaints officer to find a solution.

If your complaint is not dealt with to your satisfaction, you may be able to go to a disputes committee. The disputes committee is an independent body whose task is to settle disputes about a healthcare provider's conduct towards a client in the context of healthcare provision. This committee consists of independent members.
